INSTRUCTIONS
1. To automatically insert the current date, please click the "Get Date" button.
2. Enter your name, company and address information. (The form is set for the initial registration rate of $995.00/person. After January 15, 2003 the rate will change to $1,195.00/person.)
3. If you wish to bring a companion to the Group Evening Event click "Yes," if not click "No." Clicking yes will add the US$75 companion fee to the invoice. (If bringing a companion, please make sure the amount of $75 appears in the "Companion for Group Event" Field by clicking the "YES" button.)
4. Enter the number of tickets purchased (if any) for the GEO Awards Gala and Dinner
5. Press the "Click for TOTAL" button if the total has not automatically appeared in the "Grand Total" Field.
6. Use your Browser's PRINT Function to print a copy of this invoice for your records.

Please note: The above invoice reflects conference attendance for only one person. If you need invoices for more than one conference attendee, then you must create more than one invoice; one for each person attending.
You must repeat steps 2 through 5 for each person attending the conference.
